Support Coordination, built around your goals.
Practical help to understand your NDIS funding, identify gaps, connect services, monitor budgets and build your ability to direct your supports.
From day one to your next plan.
The right approach depends on your individual needs, plan and funded support type. These are some of the things we can work on together.
A thorough plan review
Go through your plan, goals, current supports and funded categories to identify priorities, gaps and opportunities. This is a review of how to implement your existing plan—not an NDIA reassessment.
Plan implementation
Turn funding into an achievable schedule of supports, explore provider options, arrange introductions and set up appropriate service agreements.
Regular budget monitoring
Review spending, service frequency and remaining funding at agreed intervals, including funding-period changes, so you can make informed decisions about sustainability.
Provider coordination
Help providers, allied health professionals and other supports communicate—with your consent—and address barriers such as waitlists, changing needs or service gaps.
Progress & reassessment preparation
Track progress toward goals, identify unmet needs and coordinate relevant reports and evidence for check-ins or plan reassessment where appropriate.
Capacity building
Understand provider quotes, service agreements and your options, building confidence to manage your own supports over time.
Clear communication and regular check-ins
I aim to acknowledge messages the same or next business day, and keep you informed if a question needs more investigation. Alongside regular budget and plan reviews, we’ll agree how often you’d like updates and the best way to stay in touch.
Services available
Level 1 · Support Connection
Help to understand your plan and connect with services and community supports, with a focus on getting arrangements in place and building independence.
Level 2 · Coordination of Supports
More involved assistance to put your plan into action, coordinate multiple supports, respond to challenges and build skills to manage services.
Psychosocial Recovery Coaching
Also available where suitable funding and needs align, with a recovery-oriented focus on strengths, resilience and coordinating psychosocial and mainstream supports.
Services depend on the funding included in your individual NDIS plan and suitability for your circumstances.
Funding, fees & service agreements
Direct Support Coordination is available for people with self-managed or plan-managed funding for the relevant support. If your support is NDIA-managed, you're welcome to ask about options; any service through a registered provider would need to be separately confirmed and arranged with that provider, and is not offered directly under this business.
Fees are informed by the current NDIS pricing schedule. Before support begins, we’ll confirm the applicable hourly rate and discuss any permitted travel, non-face-to-face work and cancellation charges in your service agreement. Any changes to agreed prices will be discussed with you first.
